Chart Review:

A 68-year-old man presents to the clinic with complaints of generalized weakness, fatigue, and occasional muscle cramps. His medical history is significant for hypertension and type 2 diabetes mellitus. His current medications include a thiazide diuretic, metformin, and lisinopril. Laboratory tests reveal a serum potassium level of 2.8 mEq/L (normal range: 3.5–5.0 mEq/L). Serum sodium and creatinine levels are within normal limits. Urinalysis shows increased urine output with dilute urine. A review of his medication regimen suggests that the thiazide diuretic may be contributing to his electrolyte imbalance.
